抗凝固療法中の患者におけるPCIでは, Clopidogrel追加のみでOK

WOEST trial; 既に抗凝固薬を使用している患者群において, PCIを施行した573名のOpen-label RCT.

Clopidogrel単独追加群 vs Clopidogrel + ASA併用群に割り付け, 1年以内の出血リスク, 冠血管イベントリスクを比較.

ワーファリンはINR 2.0を目標に使用.

平均年齢は70歳. PPIは其々34-39%で併用. CHADS2 score ≥2は78%.

アウトカム;

出血リスクは有意にASAを使用しない群で低くなる. 

一方で, 心血管イベントは両群で有意差無し.
出血リスクが低下する分、死亡リスクも低下している。

ワーファリン使用中の患者では, PCI後もASA+Clopidogrel+Warfarinではなく、
Clopidogrel+Warfarinでも問題なし、むしろ出血リスクは低下するので勧められるということか。

それは願ったり叶ったり。
